Clinical Liaison - Mobile Role (Plenty of Driving).
Location: Various Nursing Facilities (Daily Travel Required).

At PrimoCare , we’re reimagining what it means to connect with patients. As a Clinical Liaison , you’ll be the trusted on-site presence supporting care coordination, conducting patient assessments, and ensuring clear communication between patients, facility staff, and our clinical teams.

This is not a typical clinical role - it’s mobile, mission-driven, and perfect for someone who thrives in the field and loves working face-to-face with patients and care teams.

What You’ll Do:

• Travel daily to a network of skilled nursing facilities - a reliable vehicle and comfort with driving are essential.
• Conduct clinical assessments with senior patients and communicate findings to our provider team.
• Collaborate closely with our clinicians to support care planning and follow-ups.
• Serve as the on-site advocate for patients, ensuring they stay engaged and well-supported.
• Use technology to document visits, manage patient follow-ups, and stay connected with the team.
• Build strong relationships with patients, staff, and families across facilities.
• Be a proactive problem-solver - identify needs, act quickly, and follow through.

What Makes You a Fit:

• Clinical background (CNA, MA, or similar) with 2+ years of experience in a patient-facing role.
• Experience working with geriatric or long-term care populations is highly preferred.
• Self-starter who thrives with autonomy and can manage time effectively in the field.
• Naturally empathetic and dependable - patients and teams trust your presence and follow-through.
• Strong communicator who can build rapport quickly and navigate different facility environments.
• Comfortable with mobile tech, EHRs, and digital tools to stay organized and responsive.
